MITCHELL HIGH SCHOOL BAND FIELD TRIP FORM

I give my son/daughter permission to go on a field trip with the Mitchell High School Band.  I understand that he/she will be under supervision of certified, Mitchell County Board of Education Personnel at all times.

Trip to:                       Bakersville, NC (Christmas Parade)

Time Leaving:            5:00 PM (directly from school)  Need to be at MHS by 4:30 PM

Expected Return:      @ 7:00 PM

Additional Information:

• We will leave Mitchell promptly at 5:00 PM.

• Please leave the circle clear of cars for unloading when we return.

• Students will need to dress relatively warmly.

• Students must wear their current black band shirt in order to participate.
